It is best to contact the hotel first via phone or email since the form is not required. I would recommend politely negotiating with an employer if you’re close to the age of 18. Guests being disrespectful in hotels is a concern for some hotels. A hotel manager may let you stay without a company-wide age policy if you can demonstrate responsibility.
